TURNING KNOWLEDGE INTO ACTION

Making Texas Healthy From the Ground Up

The Texas Kidney Foundation (TKF) takes a prevention-first approach to kidney health—meeting Texans where they are and turning knowledge into action. We focus upstream, using FREE community-based screenings, early detection testing, and education to prevent kidney disease or slow its progression long before crisis care is needed.

What makes us different? TKF is consumer-led, industry-free, and community-powered. We bring care directly to neighborhoods through our medium-complexity community lab, delivering same-day results for key kidney tests and genetic results within four weeks. While others focus on awareness, we focus on activation—training Community Health Workers, building Community Health Clubs, and uniting data and prevention through Knowledge2Action 2030 to make Texas healthy again.

OUR APPROACH

Where Innovation Meets Neighbors Helping Neighbors

We meet Texans where they are—bringing early detection, education, and system navigation with follow-up.

The Texas Kidney Foundation (TKF) takes a prevention-first, community-powered approach to kidney health—meeting Texans where they are and turning knowledge into action.

We start upstream, focusing on early detection and education to prevent kidney disease or slow its progression long before dialysis or transplant is needed. Using our medium-complexity community lab and point-of-care testing technology, TKF delivers FREE kidney health screenings with same-day results for eGFR, uACR, A1C, cholesterol, and glucose—while genetic testing results are processed and delivered within four weeks through our trusted lab partners.

Through our six programs—R.E.A.C.H., Neighbors Helping Neighbors Community Health Fairs, Community Health Workers, Community Health Clubs, H.O.P.E., and Mobile App—we unite data, education, and community trust. Each program is designed to empower healthcare consumers, strengthen local capacity, and connect every participant to personalized prevention and follow-up care.

Our model goes beyond awareness—it redefines access. By combining clinical precision with grassroots connection, TKF bridges the gap between health systems and communities, ensuring that every test, every class, and every conversation moves Texans closer to lasting wellness.

At TKF, prevention isn’t just a program—it’s our philosophy.
Because when neighbors help neighbors, Texas gets healthier, stronger, and more resilient.

A DIFFERENT APPROACH

Turning Awareness Into Action Across Texas

We help Texans understand kidney health, access free screenings, and take charge of their well-being.

Most healthcare efforts begin after disease develops. The Texas Kidney Foundation (TKF) takes a different path—focusing upstream on education, awareness, early understanding of kidney health, and system navigation. We help Texans learn about the risk factors for kidney disease, understand their results, and take informed steps toward healthier living.

Unlike hospital- or industry-led initiatives, TKF is consumer-led and community-powered. We work outside the traditional system to expand access to information and free kidney health screening opportunities. By meeting people where they are—at churches, schools, workplaces, and neighborhood events—we help remove cost, transportation, and trust barriers that prevent many Texans from engaging with preventive health.

TKF also operates a community laboratory resource, supporting on-site collection and rapid reporting of kidney health indicators such as eGFR, uACR, A1C, cholesterol, and glucose. Genetic information, when offered, is processed through certified partners and delivered to participants with educational guidance—not medical advice.

Our model focuses on activation over awareness—training Community Health Workers to educate families, organizing Community Health Clubs to promote better nutrition and wellness habits, and using Knowledge2Action 2030 to connect community data to statewide learning.

At TKF, prevention isn’t treatment—it’s empowerment. We exist to help Texans understand their health, make informed choices, and build stronger communities together.

Because when health starts with people, not systems, it doesn’t just inform—it transforms lives.
